1. Call the middle school guidance office at 860-663-1241 ext. 1001 to make an appointment to meet with a school counselor.
 

2.  Print the following forms and complete applicable sections.  Submit these forms to Debra Cumpstone, our guidance secretary.

• Registration Letter

• Accepted Forms of Residency

• Registration Form

• Enrollment Agreement

• Home Language Survey

• Release

3.  In addition to the above forms, you will need the following documents in order to register your child:

• Immunization records

•  Proof of residency

•  Birth certificate

•  Valid physical on file per CT state law.
